Please complete the following form to request your Converter Repair.
**ATTENTION**
Customers must retain their tracking number to view status of their return to our facility. It is the customer’s responsibility to ensure completion of delivery.
DPC staff WILL NOT call the customer when their return has been received. We WILL contact you once work begins on your converter, if necessary.
Include a copy of completed form with your converter. We will not begin work on any converter received without this form.
Please DO NOT SHIP WITH THE UNITED STATES POSTAL SERVICE. Your converter will not be delivered.
