Repair / Return Your Converter

Please complete the following form to request your Converter Repair.
**ATTENTION**
  • Customers must retain their tracking number to view status of their return to our facility. It is the customer’s responsibility to ensure completion of delivery. 
  • DPC staff WILL NOT call the customer when their return has been received. We WILL contact you once work begins on your converter, if necessary.  
  • Include a copy of completed form with your converter. We will not begin work on any converter received without this form.
  • Please DO NOT SHIP WITH THE UNITED STATES POSTAL SERVICE. Your converter will not be delivered.
